First, what is gynecomastia and what are the causes of it?

The condition describes an abnormal growth of breast tissue. The growth is of a firm, glandular tissue which is located beneath the nipple and may even be sensitive to the touch. The growth of this tissue makes the male chest look feminine. Most importantly, this condition cannot be treated by diet or exercise. There is also pseudogynecomastia which occurs when the male chest grows in fat, which does not accompany the growth of glandular tissue. The most frequent causes of gynecomastia occur naturally due to the imbalance of estrogen and testosterone during various life stages with the addition of the use of anabolic steroids. Other causes include: use of anti-androgens, anti-depressants, drugs to manage blood pressure, recreational drugs, liver disease, and thyroid disease. There are many cases where no reason for gynecomastia can be found, and this is referred to as idiopathic gynecomastia.

Progression of gynecomastia is classified into four stages. Localized enlargement around the areola is classified as stage 1. Stage 2 is classified as moderate enlargement of the breast with some excess skin, while stage 3 is significant breast tissue and skin with an excess of glandular and fat tissue that is drooping. Stage 4 is the most advanced and classified when a breast with a feminine appearance and significant skin and tissue excess is present. Each stage also has a different treatment option via surgery.

What Actually Happens In Gynecomastia Surgery?



The official name of the surgery is a male breast reduction and its purpose is to remove excess skin and drooping breast tissue, along with fat and glandular tissue, in order to regain a flat and masculine contour of the chest.

Who is the Ideal Candidate?

The ideal candidate is a healthy male patient with stable gynecomastia for at least one year, at or near his ideal body weight, a non smoker or a smoker who will quit four weeks before the procedure, and who has a healthy mindset with realistic expectations. There is no age limit, with patients ranging from 16 year old with persistent pubertal gynecomastia, to men in their working years and their 50s and 60s.

A complete evaluation prior to surgery involves blood tests for hormones, a chest ultrasound, and a thorough assessment of lifestyle, medication, and goals for surgery. This evaluates possible hormonal causes before surgery.

What Leads to the Permanency of the Results?

This is the most common question and the answer is...yes! But with one important exception. The removal of glandular tissue is final and does not grow back. Therefore, the results are permanent. The exception would involve considerable weight gain, return to steroid usage, and other hormonal disruptions with post-surgical medication. The results are indefinite with the preservation of weight and avoidance of post-operative hormonal changes.

The satisfaction results from gynecomastia surgery indicate that the procedure is effective. A study done in 2025 across 2 cities in India analyzed the results of 967 patients who underwent a lateral port procedure and reported a satisfaction rate greater than 93% — as well as what most surgeons agree on through experience.
